Possible Causes 
MASS AIR FLOW SENSOR
O2 SENSORS
USE OF HIGH SULFUR FUEL
TAMPERING OR REMOVAL OF THE NAC
DEGRADED OR POISONED NAC
EXHAUST, EGR OR INTAKE LEAKS
