Customer Declaration On Replacement Of Front Passenger Seat, Deactivation Of ACSR, SBE, Side AIRBAG, Form - OF91.00-P-3000-02A

In response to a clearly-stated customer request, on....., the function of the side airbag, the automatic child seat recognition (ACSR), and seat occupancy recognition (SBE) has been deactivated on the front passenger side. There is an increased risk of injury for vehicle occupants resulting from this measure. There may, in particular, be

as a consequence.

This is expressly pointed out to the customer who undertakes likewise to inform other users of the vehicle. The sign "Warning! This vehicle does not have automatic child seat recognition!" has been attached to the right side of the armatures.

Copy sheet, cut off here and stick the upper section into the Operator's Manual at the beginning of the SAFETY chapter.

Exclusion of liability, exemption and measures 

  1. I hereby renounce all claims against Mercedes-Benz AG, its associated subsidiaries and its sales partners that arise - directly or indirectly - as a result of the modifications listed above and the associated, increased risk of injury.
  2. I hereby release Mercedes-Benz AG from all third-party claims and indemnification as stated under (1).
  3. Furthermore, I hereby commit myself to:
    1. To ensure that
      • the above-separated specification is always kept with the operator's manual and
      • the vehicle user/acquirer is notified about this; and
    2. the change must be immediately recorded in the vehicle documents; and
    3. the vehicle user/acquirer must be informed of the change and the associated increased risk of injury.

Deactivation of seat occupancy recognition and automatic child seat recognition
Customer:  Inquiry
  Sales
Workshop:  Provide information that decisions are made on an individual case basis
Check which version of the customer declaration "Front passenger seat replacement" is relevant
Instruct customer about the dangers mentioned in the relevant version of the customer declaration on "Front passenger seat replacement"
Customer and workshop: 
  1. Fill out the facts form,
  2. Fill out customer declaration on front passenger seat replacement
    • Leave Date modified blank (important!)
    • Customer signature + workshop stamp and signature,
  3. Customer identification
    • Copy of the ID
    • Copy of the approval certificate
    • Copy of disabled ID if a seat for a disabled person is installed

Send completed forms (1 + 2) + copies (3) by fax or email to decision-maker (GSP/TP).

Decision-maker notifying workshop of code(s) and item number of adhesive label "No ACSR"
Workshop:  Attaching "No ACSR" adhesive label to front passenger side of instrument panel
Decoding using code
Carefully document in VeDoc (process + person performing task)
Now enter the date on the customer declaration that was previously left blank
Copy the customer declaration
Cut off and stick the upper section of the original into the operator's manual (start of the chapter on Safety)
Sending lower part + copy of entire customer declaration by fax to decision-maker (GSP/TPx)
Now enter in the service booklet
Customer's obligation:  Entry in the corresponding vehicle documents (Technical Inspection Association acceptance)
Recommendation to customer:  Notify insurance company
Decision-maker (GSP/TP):  Archiving lower part of original
Archive copy of entire customer declaration at decision-maker
