Check the following: improper fluid level, high line pressure, valve body (EPC or TCC solenoid stuck or damaged, or separator plate damaged), incorrectly tightened valve body bolts (cross-leaks), front band or servo damaged or out of adjustment, or intermediate band or servo damaged or out of adjustment.
