Possible Causes

SENSOR LENS OBSTRUCTION
DAMAGE OR MISSING SENSOR LENS
ACC MODULE

For gasoline engines, always perform the Pre-Diagnostic Troubleshooting procedure before proceeding. Refer to  DTC-Based Diagnostics/MODULE, Powertrain Control (PCM) - Standard Procedure  .
